Community First Services and Supports (CFSS) is a Minnesota program that provides flexible options for individuals who need assistance with daily living activities and community participation.
CFSS gives individuals greater choice and control over how their services are delivered while promoting independence and self-direction.
Our CFSS team assists participants with:
- Daily living activities
- Personal care support
- Community engagement
- Goal planning and achievement
- Independent living support
- Service coordination
- Family and caregiver support
